Our records staff is asking that requests for a copy of a police report be submitted with the attached fillable form. Be sure to include a copy of your government issued ID within the form. You can attach your ID by clicking inside the yellow Upload ID box.

When requesting a copy of a police report:

• This form is not for law enforcement or allied agencies. Please fax your request on department letterhead.
• Requests for body camera and in-car video footage are only made available by subpoena from a court.
• You must be an involved party to request a copy of a police report/incident.
• If you are an attorney requesting a copy of a report, you must mail in your request on official letterhead and include a copy of a signed authorization from your client granting you permission to obtain a copy along with a self-addressed stamped envelope.

Reports will be mailed via U.S. Postal Service in the coming weeks. Please note the report request form will not send if the following mandatory fields are not completed:

• Date of Request
• Name
• Address
• Phone Number

After filling out the form, press the orange Submit button which will automatically open your email client and attach the completed form to an email draft. Be sure to send the email after confirming the Report Request form is attached to your email.

You will need to use a PDF Reader to fill out the form, such as Adobe Acrobat Reader or Adobe Acrobat DC. You will not be able to fill out the form using an in-browser PDF viewer.

Report Request Workflow

1. Download and install Acrobat Reader or already have it installed on your machine.
2. Right click on the report request form below and Save As to a location on your machine.
3. Once you have downloaded the form, make sure to open it with Adobe Acrobat or similar.

You may need to change how your system handles PDF files. In the event double-clicking on the file does not open the file in Adobe Acrobat, right-click on the file and select Open With, and then choose Adobe Acrobat Reader.

To obtain a police report, a report request form must be completed and signed by the requesting party. These forms are available at the Records counter at the Pacifica Police Department. The Records office is open with the exception of some holidays:

• Monday: 8:30am to 5:00pm
• Tuesday: 8:30am to 5:00pm
• Wednesday: 8:30am to 7:30pm
• Thursday: 8:30am to 5:00pm
• Friday: 8:30am to 1:30pm

For your convenience, you may request a report in writing. Please include a self-addressed, stamped envelope with your request. Your request should include your name, date of incident, report number (if known) and enough information so that we can comply with your request.

If you are not entitled to receive a report you will be notified in writing. The department is not currently charging a fee for a police report.

If you do not wish to come to the Police Department, effective April 1, 2010 most vehicle accident reports may be obtained online for a small fee. Please follow the link below to Lexis Nexis and follow the directions to access you report. To search for an accident you will need the report number, date of the accident, and name of the involved party (accidents involving special circumstances, fatalities, and some hit and runs will not be available online). Please allow a minimum of 7 days after the accident before attempting to obtain a report. If you have any questions call Police Records at (650) 738-7314.
